I am a diver in Oregon who has heard of this through other divers. I was not there, do not know the divers involved and have no information other than what's in the articles (poorly written, as they often are about dive incidents). I posted to ScubaBoard Monday, and the most information that has surfaced is in the comments section of the Oregonian article where a family member has posted. (Certified diver, good physical shape, fair number of dives including night under her belt.)

I'm posting in hopes more information will surface. As a community, I feel incident awareness and analysis is an important reminder that we are only as safe as our skills are strong. Perhaps someone can learn something so this doesn't happen to someone else.
